Please select the testing date on the calendar below to view and choose from the available appointment times. Enter in the patient’s name parent/guardian's and contact information above the Consent Form. This is who we will be contacting with test results.
NOTE: Screenings are limited to youth ages 12-22 only.
There are multiple appointments available per time slot. If you receive a confirmation and the time is still listed as available, you are scheduled for that time but there's an additional appointment that can still be scheduled for that time.

Living for Zachary is able to provide free youth heart screenings to the public due to the generosity of our donors. Appointments are booked on a first-come, first-serve basis and fill quickly. If you do NOT attend the appointment for which you are booked or cancel on the day of the event, you will be charged with a $25 no-show/late cancellation fee.

A parent or guardian must be present for youth under 18 years of age during the Living for Zachary Heart Screening.
