NOTE: There are multiple appointments available per time slot. The time will be removed from the available list once ALL are filled. If you receive a confirmation and the time is still listed as available, you are scheduled for that time but there's an additional appointment that can still be scheduled for that time.
Living for Zachary is able to provide free youth heart screenings to the public due to the generosity of our donors. Appointments are booked on a first-come, first-serve basis and fill quickly, with many events having waitlists. As you register, you will be asked to submit your credit card information to hold your screening appointment.
Your card will NOT be charged if you attend your screening appointment or cancel prior to the event date. To cancel your appointment prior to the event date, please email please email heartscreenings@livingforzachary.org or call 972-478-5959 so your appointment can be re-booked with a new patient. A parent or guardian must be present for youth under 18 years of age during the Living for Zachary Heart Screening. Thank you for your help in ensuring more youth can be screened through Living for Zachary! Both parent and child must wear a mask before entering the building and during the screening. Thank you. |
